We assess medical priority for physical and mental health conditions based on evidence from your GP or specialist. If you believe you, or a member of your household, have a medical condition which is impacted by your housing, we will need to see evidence to support your application. That includes evidence that:

• There is a clear risk to your health that would be removed by moving home
• Someone could avoid moving to a residential or nursing home if they lived in a more suitable home
• Your home needs to be adapted but the works cannot be carried out
• The layout or design of your home makes it difficult to live independently, has led to someone being housebound, or is stopping discharge from hospital
• A medical condition is likely to get significantly worse and have an unacceptable impact on quality of life because of your current housing.
